Healthcare Directives

Texas law now allows a person's signature to be acknowledged by a notary instead of witness signatures and for digital or e-signatures on the following Out of hospital D-N-R, Medical POA, and Directive to physicians.

Jurats/Acknowledgments

A Jurat is where the signer swears under oath or affirms the truthfulness of the document brought before a notary.  An Acknowledgement is where the signers identity has been verified to declare that they have willingly signed the document before a notary.
